REGISTRATION

I, the undersigned parent/guardian, hereby consent to my child's participation in Thorney Football Club and confirm that all information provided on this form is accurate to the best of my knowledge.

I understand that participation in football activities involves inherent risks, and I release Thorney Football Club, its organizers, coaches, and volunteers from any liability for injuries or accidents that may occur during club activities.

I have read and understood the clubs code of conduct for both parents and children. I will always adhere to the code of conduct and acknowledge this is mandatory. If the code of conduct is not upheld I accept this could lead to suspension or removal from the football club.

I understand that it is my responsibility to ensure that payments are made by the designated date set by the football club.
